In consideration of permitting my child named above to participate in rehearsals, performances, and all other activities of the San Diego Children's Choir between September 1 and August 31, we, guardians of said minor, forever release, acquit and discharge the San Diego Children's Choir and its employees and representatives from any and all liability. The undersigned hereby agree(s) to indemnify and save harmless the above parties from all loss, damage, costs and expenses, by reason of any claim, demand or suit made on behalf of said minor child. // En contraprestación por permitir que mi hijo participe en los ensayos, presentaciones y todas las demás actividades del coro infantil y juvenil, llamado ¨San Diego Children’s Choir¨, entre el 1 de septiembre y el 31 de agosto, nosotros, los guardianes de dicho menor, liberamos y absolvemos a dicho coro, a todos sus empleados y representantes de cualquier responsabilidad. El abajo firmante se compromete a indemnizar y no responsabilizar a las partes mencionadas en la parte superior de este documento de ninguna pérdida, daño, costo y /o gasto causado por razón de cualquier reclamación o demanda hecha a nombre de dicho menor.

This form gives permission for the use of image, pictures and recordings of my child, listed above, by the San Diego Children's Choir without compensation, except as may be agreed upon in advance for certain projects. This permission slip is a waiver for all choir functions regardless of payment or other benefits to the Choir. // Este formulario le otorga permiso para el uso de imágenes, fotos y grabaciones de mi hijo, que se enumeran más arriba, por el Coro de niños llamado “San Diego Children’s Choir ¨sin compensación alguna, salvo que se haya convenido de antemano para ciertos proyectos. Este permiso es una renuncia para todas las funciones del coro independientemente del pago u otros beneficios para el coro.
